8:30 am – 11:30 am Patient Centered Medical Home (2.75 CME credits available)
The patient centered medical home (PCMH) is an approach to providing comprehensive primary care to adults, youth and children. This approach emphasizes a continuous relationship with a personal physician coordinating care for both wellness and illness.
Throughout the morning we'll offer practical guidance on how to transform your practice into a working PCMH, or how to improve your existing PCMH.
First, engage with a panel of PCMH adopters on the innovative reforms that help their practices run smoother. Then join us for our next two sessions that will show you how to execute two of the most exciting changes associated with the patient centered medical home: Group Visits and Same Day Appointment Access.
The morning will end with a special session from the Purdue Health Information Technology Regional Extension Center (REC). Director of the REC Monica Arrowsmith will be speaking on how the REC is available to give you step by step assistance to achieve Meaningful Use no matter your stage of Health Information Technology implementation or understanding. Learn how your practice can earn the federal stimulus dollars it deserves. Monica will be answering your questions and discussing:
- Discounts and vendors that are available through the REC (with prices)
- Costs for consultations
- What services the RECs offer
- Who qualifies for the meaningful use ARRA stimulus dollars
- How to apply, and whether your office should chose the Medicare or Medicaid program
- How to report on the measures required to receive the ARRA stimulus dollars
- How the payments will be determined and made to physicians
REC services include:
- Assessment of practice IT and recommendations for improvements
- Implementation assistance with any EMR
- Assistance in applying for federal ARRA meaningful use money
- Assistance in qualifying for meaningful use

A targeted team-based approach to practice transformation in the family physician's office.
Patient Program Learning Objectives:
1. Identify the differences between African American and Caucasian hypertensive patients
2. Identify and address physician barriers to blood pressure control in African American patients
3. Appraise and address patient barriers to optimal blood pressure control in the African American patient in a culturally sensitive manner
4. Develop strategies to improve compliance to medication and lifestyle modifications in the African American patient
5. Implement strategies to improve the percentage of African American hypertension patients whose blood pressure is controlled
6. Transform practice to work in the new model of care

8:30 am – 12 noon MC-FP SAM Study Group: Childhood Illness
Our SAM Study Groups feature reference slides showing sources used in each of the 60 questions in the ABFM’s Self-Assessment Modules, and an overview of the MC-FP process and how this study group fits into it.
- Explore the topic via interactive discussions.
- Complete the Knowledge Assessment portion of your MC-FP Part II Self-Assessment Module and we report the answers to the ABFM
- Earn 15 CME credits after this session by completing the online Clinical Simulation

Topics will include:
- Caring for Refugees - Maurice Henein, MD
In 2008, 1552 international refugees arrived in Indiana. The number of refugees coming into Indiana is expected to increase each year. This population has distinctive needs that may fall out of a family physician’s comfort zone. Issues such as reviewing pre-departure State department exams, working closely with the local health department and resettlement agencies are important in providing excellent health care to this population. This presentation will focus on outlining the refugee resettlement process and the medical issues that need to be addressed such as screening for: Tuberculosis, Hepatitis (A, B, C), HIV, Syphilis, Lead poisoning, Intestinal Parasites, Anemia and other medical issues. The use of appropriate medical interpreters is necessary in most cases. Mental health issues can be a challenge, but must always be considered in groups who have fled their homeland for various reasons. Caring for this underserved population group is extremely rewarding and challenging. It is a task that family physicians are most qualified to handle.
Learning Objectives:
1. Be aware of the community, state and federal resources available in caring for newly arriving refugees
2. Be able to anticipate what medical issues need to be addressed at a newly arriving refugees’ first office visit
3. Understand what the most cost-effective methods are in screening for infectious diseases that may be present in this patient population
- Skin Manifestations of Collagen Vascular Disorders - Patricia Treadwell, MD
Learning Objectives:
Gain knowledge regarding the cutaneous findings in the following disorders: systemic lupus erythematosus, neonatal lupus erythematosus, morphea, juvenile dermatomyositis, Kawasaki disease, and Henoch-Schonlein purpura. A discussion of the work-up for these disorders will be included.
